Have you completed & supplied your employer a self certification ( SC1 form ) for 1st – 7 days of your Absence?

( note our fit note is not valid for 1st 7 days & will be refused ) 

DO NOT contact the surgery for your vaccination status. 

GP’s cannot provide letters showing your COVID-19 vaccination status. 

Proof of vaccination status will be available from MAY 17th through the NHS app.
